I definitely would go with Kath's recommendations as well, but a couple of other good ones are Hatha Yoga Illustrated and The Yoga Bible.

David Swenson has a spiral bound one that shows the poses with and without modifications. It is nice because it is spiral bound so it lays flat. I am at work and don't remember the exact name.
