Being called lady wouldn't bother me...that's easy enough to correct to Lady Vol Fan
But the other two things I would address with his supervisor even though I'm not creeped by them.
Primarily because professionalism isn't taught in schools as it should and needs to be developed.
If he knows his craft, you can help him be better by alerting him to behavior that makes some people uncomfortable or crosses lines. (The discussion with your husband should not happen, too. You're not a child at daycare who needs the teacher to talk to your father about your day )
As a side note, this younger generation (if he's younger) looks up people immediately they come in contact with so although that wouldn't bother me ..I would prefer that happen during the intake interview. Perhaps he did it to build rapport because it's important to him to build that. I would want his supervisor to tell him that's an unnecessary step in his treatment protocol.
