That's a big problem for me too. Ugh.

I like Erich Schiffman's Backyard Beginner - lots of feel-good stretches for the traps, shoulders and chest.

I also try to use my foamroller and to do some of the poses from Elaine Petrone's Miracle Ball Method.

When all else fails, a massage works miracles

I swear by my foam roller and Petrone's Miracle Ball. The Miracle ball is especially great for this area. Jill Miller has some nice clips on how to self massage this area with a ball:

You're welcome, Fuzzie and yahoo.

The trap stretch in the link is used in p90X, and it's a fantastic stretching exercise. I do this one before I go to the floor to do my foam rolling and ball rolling.

Speaking of tight traps I just had an awesome massage to work those babies out! If I could afford it I swear I would get massages weekly! STS has been doing a number on the tightness in my neck/shoulders/traps! I'll have to try the stretches too!

My traps have been tight as well.
I'm finding some relief while sitting in the chair at my desk by tilting my head to the side and pushing down on the top of the shoulder (of the side I'm tilting away from) with my opposite hand, or even grabbing my lateral delt and 'pulling'. Similar to the stretch in the photo, but adding the push/pull.
